General Waiver and Release for Jersey City Ballet Theater Corporation also d/b/a as Jersey City Ballet

I, being the registered party or parent guardian of the registered party, understand that dance instruction/performance requires physical exertion and risks. I acknowledge that injury may result during the following mentioned all activities including but not limited to formal or informal performances, rehearsals, classes, transportation, and appearances including but not limited to as a result of physical contact with other dancers, instructors, fixed or movable objects, or flooring.

 I represent that I am medically and physically able to safely participate in all activities; at, or in connection with, Jersey City Ballet Theater Corp and meet the physical demands thereof. I agree to assume all risks associated with participating in all activities including but not limited to dance instruction, workshops, rehearsals, photo shoots , appearances, and performances at or in connection with Jersey City Ballet Theater Corporation

If, at any time hereafter, I have any medical or physical limitation or condition that might affect my ability to safely participate in all the above-mentioned activities or meet the physical demands required thereof, I will make such limitations and conditions immediately known to Jersey City Ballet Theater Corporation.

I consent for any and all necessary emergency medical care as such arises and agree to individually provide for any future medical expenses which may be incurred due to injury as a result of participating in above mentioned all activities in connection with Jersey City Ballet Theater Corporation.

I have read the above and voluntarily participate in any and all activities associated with Jersey City Ballet Theater Corporation and assume all risks and hazards incidental to the conduct including but not limited to the mentioned all activities.

Photo Release: The below-signed participant will have final approval of any photos posted in association with Jersey City Ballet Theater Corporation and has permission to use photos/videos/sound bites taken for publicity and/or as a result of participating at or in connection with and JCBT Corp.

By signing and/or acknowledging this waiver, I forever release, absolve, Indemnify & hold harmlessJersey City Ballet Theater Corporation, its employee’s affiliates, sponsors, officers, teachers, coaches supervisors,  contractors,  board members, and volunteers from all claims derived from any injury that may be sustained by me while participating in the mentioned all activities including classes, rehearsals, events, appearances or performances.

This Waiver is intended to be as broad and inclusive as is permitted by the laws of the state of New Jersey.

Jersey City Ballet cannot be held responsible for lost or stolen articles
